Corrections policy

Health information evolves, and published material should be corrected when evidence, safety information or factual understanding changes.

Material corrections

When a factual, safety, evidence or sourcing error is identified, the affected record should be moved out of Published status while the correction is made when the issue could materially affect interpretation.

Review renewal

Material content or evidence changes alter the revision fingerprint, so earlier approvals no longer satisfy the publication gate. Required reviews must be completed again.

Dates

Pages display review dates where available. Sitemap modification dates are based on actual record changes rather than being refreshed merely because the sitemap was requested.
